Mercy REACH offers for alcohol, drugs and tobacco treatment. Our program is compassionate, non-judgemental, caring for people struggling with substance use.
SUPPORTIVE CARE: Case management and peer support, supporting a “warm handoff” approach across care transitions as well as promoting patient engagement, activation, and self-care over the long-term in order to reduce the potential for relapse; transportation assistance; linkages with local AA, NA, faith-based, sobriety support programs; and educational programming (modification strategies for high risk behaviors).
